Five Components of the Optimal Diabetes Care Measure
Optimal Diabetes Care is made up of five measures. The measures are:
• Hemoglobin A1c < 7%
• Blood Pressure < 130/80
• Low-Density Lipoprotein (LDL) Cholesterol < 100
• Daily Aspirin Use
• Tobacco Use
